An Outreach Mobile Skin Clinic was held in the flood affected area of Muzaffar Garh under the auspices of the Pakistan Society of Teledermatology. This was a collaborative venture by the volunteers from Canada, United States, and Pakistan. A series of such clinics has been launched by the society ever since the severe flooding struck Pakistan.

Mobile Skin Clinic in Muzaffar Garh

A mobile skin clinic was organized by the Pakistan Society of Teledermatology in the flood affected area of Muzaffar Garh on Friday, October 1, 2010. Over 400 patients suffering from skin disease were examined and treated. Medicines were provided free of cost. We are highly grateful to Mr. Abdul Hameed and Dr. Maqbool from Muzaffar Garh for their whole hearted efforts to make this mobile skin clinic possible. Our team comprised of Dr. Aisha Sethi, Dr. Shahbaz Janjua, Mr. Abdul Rahman, Mr. Shabbir Ahmad, and Mr. Waseem Akhtar.

Teleconsultation services have also been provided for five selected medical camps employing both store and forward and real time teledermatology. This is a continued effort. I am curious to learn if there are anywhere in the world better financial models of practicing teledermatology. Let's be clear - even in the store & forward modality, it is hard work and requires time and energy. However, there are no good reimbursement models in the US.

Efficacy of oral antibiotics to treat secondary syphilis

The lesions of Secondary syphilis are usually red and scaly, typically found on the chest and the palms of the hands.As the lesions age they can become less scaly and more polymorphic becoming more papular and sometimes pustular and pigmented.They usually begin 6 to 8 weeks after the penile ulcer or chancre and can last days or months. Treatment of choice remains single dose of benzathine penicillin 2.4 mega units given intramuscularly. Which oral antibiotic have been found equally effective in case of penecillin hypersensitivity? Acrodermatitis continua

Acrodermatitis continua is a chronic inflammatory disese characterized by multiple pustules with scale on an erythematous base usually restricted to a distal location on one or two digits. The pustules may coalesce to form lakes of pus, and, over time, they may spread proximally to involve the dorsal aspects of the hands, forearms, and feet. Pustulation of the nail bed and nail matrix often is associated with onychodystrophy. Acrodermatitis continua can be resistant to treatment. How do you manage this condition? View images

Eczema herpeticum

Eczema herpeticum, also known as Kaposi's varicelliform eruption, represents widespread cutaneous HSV in patients with pre-existing skin disorders. Atopic Dermatitis is by far the most common underlying skin condition. Other skin conditions that predispose patients to this primary HSV infection include: Darier disease, pemphigus foliaceous and icthyosis vulgaris. The differential diagnosis may include widespread impetigo. The diagnosis is made by history and the clinical presentation. Tzanck preparation is a useful confirmatory test, and viral culture may also be helpful.
Eczema herpeticum is treated with oral acyclovir 200 mg 5 times daily. Burow's soaks 2 or 3 times daily may be useful for vesicular areas. Early and aggressive therapy of secondary infection is necessary. Infants with high fever, poor oral intake, and evidence of secondary infection, may require inpatient management with intravenous acyclovir or antibiotic coverage. How often do you see eczema herpeticum in your practice? Can you appreciate an underlying skin condition in all such cases? View images

Acrodermatitis enteropathica

Acrodermatitis enteropathica is an autosomal recessive disease due to a partial disorder of intestinal zinc uptake. Signs usually appear within the first months of life, with a characteristic cutaneous rash that is symmetrical, located around the body orifices, behind the ears, and on hands, feet, and head. Zinc deficiency may be due to inadequate intake, malabsorption, excessive loss, or a combination of these factors. If treated early, most of the symptoms are reversible and usually leave no sequelae. Untreated patients usually die within the first few years of life. They have severe growth retardation, dermatitis, alopecia, secondary bacterial and fungal infections, and neurological and behavioral changes. Treatment is essentially with dietary or I/V supplementation. Dietary supplementation with 2 – 3 times the recommended daily allowance, 30 – 55 mg/day of elemental zinc dramatically reverses the manifestations within hours to days. View Images

Polymorphous light eruption

Polymorphous light eruption(PMLE) is an abnormal, recurrent, delayed reactions to sunlight, ranging from erythematous papules, papulovesicles, and plaques to erythema multiforme like lesions on sunlight-exposed surfaces. Only one clinical form is consistently manifested in any one patient, but combined morphologies may also result.. PMLE affects females 2-3 times more often than males. The eruption typically occurs in spring typically at the onset of a vacation in a sunny place or at a high altitude and disappears without scarring by the time the patient returns home. I have seen good response to oral beta carotene, antihistamines and topical steroids. How do you manages such cases in your practice? View images

Epidermodysplasia verruciformis

Epidermodysplasia verruciformis is a rare, lifelong disorder of autosomal recessive inheritance. It is characterized by chronic infection with human papillomavirus that manifests as widespread skin eruptions of flat-to-papillomatous, wartlike lesions and reddish brown pigmented plaques on the trunk, the hands, the upper and lower extremities, and the face. The lesions have a potential to transform into malignant carcinomas during the fourth and fifth decades of life. Palmoplantar keratoderma, sclerodactyly and nail dystrophy

Scleroatrophic syndrome of Huriez is a rare congenital genodermatosis of autosomal dominant inheritance, characterized by scleroatrophy of the hands and feet, nail hypoplasia, palmoplantar keratoderma and hypohidrosis. This 16-year-old boy was born with mild PPK that slowly progressed and worsened alongwith the development of sclerodactyly and nail dystrophy. The development of aggressive squamous cell carcinoma of the affected skin is distinctive feature of the syndrome, occurring in >15% of affected individuals. SCC in Huriez syndrome is characterized by early onset, mostly in the third to fourth decade of life, and by early metastasis. What treatment modalities do you suggest for the severe keratoderma associated with this condition? View images
